Case Management   Case Management

Family & Children Services’ Case Management program specializes in working with youth up to age 18 and their families and young adults ages 18-26 yrs. Case Management Services empower individuals/families to successfully participate in a community environment by providing services that are responsive, respectful, and adaptive to individual needs and preferences.

Case Management Services are flexible and strength based to meet client needs. Efforts are focused on empowering individuals with multiple service needs so they may achieve their goals, direct their own lives and reduce dependency on service providers.

FACT

Family & Community Treatment (FACT)

FACT (Family And Community Treatment), an intensive, home-based treatment service, works with the entire family of a youth experiencing a serious emotional disturbance and having difficulties in multiple areas of life. Services may include individual, group, family, couples and play therapy, parenting education and support, 24-hour on-call availability, advocacy and referrals to community supports.

The Mobile Crisis Response (MCR) program is a community-based service offering immediate response, 24-hours per day, seven days per week, for youth in Kalamazoo County facing a mental health crisis and their families. Services include crisis intervention, clinical assessment and coordinating short-term crisis placements.

Families First
Families First is an intensive 4-6 week voluntary program serving families whose children are at-risk of out of home placement. Families receive an average of 10 hours face -to-face contact per week. Interventions focus on addressing needs, parenting issues and connecting families to community services.
